DEPARTMENT OF ORAL PATHOLOGY & MICROBIOLOGY Clinical Case Reports

Histopathological & Clinical Case Reports

Subharti Dental College & Hospital, Meerut

Detailed documentation of rare, challenging, and significant oral biopsies, tumor resections, odontogenic neoplasms, oral malignancies, and fibro-osseous lesions diagnosed and managed by the Department of Oral Pathology & Microbiology.

Odontogenic Tumor

Case Report 1: Plexiform Ameloblastoma

Patient: 18 Years Male
Clinical History & Presentation

Pain in right upper tooth region since 15 days, discomfort in eating food, and mild facial swelling on right side.

Radiographic / OPG Findings

OPG radiograph revealed the presence of impacted maxillary third molar on the right side.

Histopathological Examination

Multiple bits of soft tissue specimen received for histopathological confirmatory diagnosis. Examination revealed the presence of ameloblast-like cells arranged in a network enclosing central stellate reticulum-like cells. Few areas demonstrated pseudoglandular arrangement of ameloblast-like cells. Ghost cells were also evident.

Final Histopathological Diagnosis Plexiform Ameloblastoma
Treatment / Clinical Outcome Wide Surgical Enucleation

Case Report 2: Well Differentiated Oral Squamous Cell Carcinoma (OSCC)

Patient: 32 Years Male
Clinical History & Presentation

Painful ulcer in left lower back tooth region since 1 month with chewing difficulty. History of smoking bidi & chewing gutkha with poor oral hygiene. Intraoral ulceroproliferative, exophytic growth in lower left buccal vestibule (extending 34 to 37).

Radiographic / OPG Findings

OPG revealed radiolucency extending from left second premolar to second molar along with evident root resorption.

Histopathological Examination

• 4x: H&E stained section shows connective tissue stroma being invaded by numerous dysplastic epithelial islands. • 10x: H&E stained section shows well differentiated dysplastic island epithelium with moderate keratin pearl formation. • 40x: Dysplastic features including cellular pleomorphism, hyperchromatism, altered N/C ratio, and moderate inflammatory infiltrate.

Final Histopathological Diagnosis Well Differentiated Oral Squamous Cell Carcinoma
Treatment / Clinical Outcome Radical Neck Dissection followed by referral for Radiation Therapy

Case Report 3: Rare Juvenile Ossifying Fibroma (Psammomatoid Variant)

Patient: 19 Years Female
Clinical History & Presentation

Reported to Subharti Dental College & Hospital with chief complaint of facial swelling over right side of face since 6 months.

Radiographic / OPG Findings

Diagnostic workup and radiographic imaging suggestive of benign fibro-osseous expanding lesion.

Histopathological Examination

H&E stained section shows extremely fibrocellular connective tissue stroma showing predominantly plump fibroblasts and few collagen fibres. Numerous eosinophilic ossifications are seen within the stroma. Concentric lamellated calcified structures with brush borders resembling psammoma bodies are present. Bony trabeculae are also evident.

Final Histopathological Diagnosis Juvenile Ossifying Fibroma (Psammomatoid Variant)
Treatment / Clinical Outcome Complete Surgical Excision & Histopathological Confirmation
